1. Identify the exact field

Do not start by repeatedly changing the same spelling. First confirm which representation the field requests.

姓 / 名: family name / given name
セイ / メイ: family-name reading / given-name reading, usually Katakana
フリガナ: phonetic reading; the form’s example reveals whether it expects Katakana or Hiragana
ローマ字氏名: name in Latin letters

2. Check script, width, and separators

A validator can distinguish characters that look nearly identical. Ordinary DAVID and full-width ＤＡＶＩＤ are different Unicode characters. The same applies to a normal space and the Japanese full-width space.

Remove middle dots, hyphens, or spaces only when the form’s example or error requires it. Do not delete them blindly from an official spelling.

3. Match the authoritative record

Katakana spellings are not unique. A bank, carrier, employer, or government service may compare the entry with an existing record. Use the passport, residence card, application record, or account spelling explicitly named by that service rather than choosing a new transliteration.

A five-minute troubleshooting order

Turn off or clear autofill for the rejected fields.
Split family and given names according to the labels, not visual order.
Convert only the field that requests Katakana or full-width text.
Try the separator shown in the form’s example; if none appears, remove the separator.
Compare every character with the name already registered for that service.
